Write the board, tier, paper and topic. 'Science revision' is too broad; 'AQA Combined Science Higher, Biology Paper 1, infection and response' is actionable and protects against revising content from the wrong course.
Plan for the next grade boundary skill
A student working toward a secure grade 4 may need dependable core methods and completion. A student moving from 7 to 9 may need unfamiliar contexts, precision and evaluation. The practice should match the current gap without limiting ambition.
Make English practice observable
Do not only reread model answers. Annotate a fresh extract, choose evidence, plan the response and write under time. Feedback should identify the next controllable improvement, such as analysing an effect rather than adding more quotations.

Topic practice builds a method; mixed practice teaches recognition. After learning a skill, mix it with older topics so the question no longer announces which method to use.
